TLDR Early recognition and management of connective tissue diseases like lupus are crucial in young patients.
A previously healthy 21-year-old African-American woman presented with acute vision changes and neurological deficits, leading to the diagnosis of systemic lupus erythematosus (SLE) and Libman-Sacks endocarditis. Her symptoms included nonscarring alopecia, thrombocytopenia, and positive autoimmune markers. Despite negative infectious and hereditary coagulopathy workups, her condition was managed with anticoagulation therapy and prednisone, and she underwent mitral valve repair. This case highlighted the importance of considering SLE in young patients with embolic stroke for early recognition and treatment to improve outcomes.
