A 25-year-old Malay fireman with nevus comedonicus, characterized by a linear group of hyperpigmented comedo-like papules on the right side of his neck, presented for cosmetic reasons. The skin biopsy revealed dilated hair follicles filled with keratin, while the interfollicular epidermis appeared normal. Initial treatments with keratolytics and topical tretinoin were ineffective, but the lesions were successfully excised.
