The study evaluated the effectiveness of excision with primary closure for treating extensive acne keloidalis nuchae (AKN) in 25 young, healthy black men. The results showed good to excellent cosmetic outcomes with no complete recurrences of acne keloids. However, 15 patients developed tiny pustules and papules, and 5 developed hypertrophic scars, which were successfully treated with steroids. The study concluded that excision with primary closure is an excellent treatment for extensive AKN, with extremely large lesions requiring multiple stages for optimal results. Cold-steel excision was preferred over electrosurgical excision due to less postoperative pain and lower risk of dehiscence.
