This study reports the successful use of a combined approach involving repeated autologous lipostructure and follicular unit extraction (FUE) hair transplantation in a 40-year-old woman with morphea en coup de sabre, a condition causing scarring alopecia. Over 5 years, the patient underwent four fat grafting sessions using the Coleman technique, followed by FUE hair transplantation with 1,600 grafts nine months after the final fat grafting. The procedure resulted in rapid and stable hair regrowth by the third month, with excellent aesthetic outcomes and high patient satisfaction at 12 months. This case suggests that fat grafting can enhance hair graft survival by improving dermal quality and vascularization, offering a promising treatment for scarring alopecia in stable patients.
