TLDR Improving skin barrier and using antifungal treatments can help manage dandruff.
The review article discussed the multifactorial etiology of dandruff, emphasizing the role of the commensal yeast Malassezia, sebum production, and individual susceptibility. It highlighted that a compromised stratum corneum (SC) barrier leads to increased transepidermal water loss, subclinical inflammation, and higher susceptibility to irritants. The review proposed that improving SC integrity alongside antifungal treatments could be beneficial for managing dandruff. Effective antidandruff treatments aim to restore scalp health by reducing Malassezia levels and facilitating flake removal. Advanced shampoos combining mild surfactants, direct barrier lipid replenishment, and antimicrobial activity were suggested to optimize scalp health in individuals with dandruff.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

The user has been on finasteride for 4.5 years and minoxidil for 10 years, maintaining hair but not regrowing much on the crown and temples. They are considering using a dermastamp more frequently and possibly trying RU58841, but are hesitant about dutasteride due to concerns about side effects and DHT suppression.

Tretinoin may enhance minoxidil's effectiveness for those not responding to minoxidil alone, but it could reduce efficacy for some who are already responding to minoxidil. Tretinoin requires careful use, including skin moisturizing and sun protection due to increased sensitivity.
