TLDR Isotretinoin effectively improved severe pityriasis rubra pilaris with complications.
A 62-year-old woman with severe pityriasis rubra pilaris, initially misdiagnosed as psoriasis, developed complications including Kaposi's varicelliform eruption and a methicillin-resistant Staphylococcus aureus (MRSA) infection. Despite initial treatments with topical corticosteroids and oral methotrexate, her condition worsened, leading to full-body erythroderma, hair loss, and nail changes. A comprehensive work-up ruled out hematologic and oncological disorders. The patient was treated with intravenous corticosteroids, antibiotics, antiviral drugs, and isotretinoin. Her condition significantly improved, with almost clear skin 43 weeks after starting isotretinoin, highlighting its effectiveness and safety for treating pityriasis rubra pilaris with such complications.
