Semen Cryopreservation in Testicular Cancer: Before or After Orchidectomy?

    TLDR Post-orchidectomy semen has better DNA integrity for fertility treatments.
    The study conducted at 'Sapienza' University of Rome on 176 testicular cancer patients evaluated the optimal timing for semen cryopreservation by analyzing semen quality before and after orchidectomy. Results indicated a decline in semen parameters post-surgery, with decreased total sperm count and motility, and increased abnormal forms. Hormonal changes included increased FSH and LH levels, while inhibin B decreased, suggesting reduced spermatogenesis. Pre-orchidectomy FSH levels and BMI were predictive of post-surgery semen quality. Despite improved sperm DNA fragmentation post-orchidectomy, the study recommends pre-operative cryopreservation due to overall deterioration in semen quality. A personalized approach to fertility preservation is emphasized, considering individual semen parameters, DNA integrity, and testicular health.
