TLDR The review helps improve diagnosis and treatment of challenging hair disorders.
The British Hair and Nail Society, formed in 2011 with 133 members, conducts an annual grand round to review challenging hair disorder cases submitted by British Association of Dermatologists (BAD) members. Since 2018, 26 cases (10 children, 16 adults) have been reviewed, with a median patient age of 33 years. The cases included equal numbers of scarring (13) and nonscarring (13) alopecia, with alopecia areata and folliculitis decalvans being the most common diagnoses. The review altered diagnoses in 38% of cases and suggested novel treatments like naltrexone and manuka honey. The initiative supports complex diagnostic and therapeutic decisions and encourages future submissions.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
