TLDR Retrograde Alopecia is fairly common in men with severe hair loss and needs careful examination for treatment and safe hair transplantation.
The study "Retrograde Alopecia: Prevalence, Patterns, Dermoscopic Features among Egyptian Men: A Cross Sectional Study" examined 1000 patients from Al Hussein hospital's dermatology department. The study found that 50 out of the 1000 patients were affected by Retrograde Alopecia (RA), a type of hair loss that occurs in the nape of the neck and can extend to the occipital region or above in the scalp. This condition is often associated with Androgenetic Alopecia (AGA), and as the severity of AGA increases, RA becomes more frequent, prominent, and severe. The study also developed a grading system for RA and linked it with eligibility for hair transplantation for AGA. The researchers concluded that RA is fairly common among patients with severe AGA, and that proper clinical and dermoscopic examination is crucial for accurate diagnosis, treatment, and for ensuring a safe donor area for hair transplantation.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
