    A 79-year-old female with a large Proliferating Trichilemmal Tumor (PTT) on the occipital region was treated with neoadjuvant radiotherapy due to her advanced age and the tumor's size. The treatment resulted in a near complete response and good cosmetic outcome. This case highlights the effectiveness of radiotherapy as an alternative to surgical excision for PTT, especially in patients who are not ideal candidates for surgery.
