    Trichilemmal carcinoma is a rare skin cancer with an incidence of 0.05%, often affecting older individuals in sun-exposed areas and originating from the hair follicle's external sheath. This case study involves a 60-year-old male with a scalp swelling initially misdiagnosed as a sebaceous cyst. A biopsy indicated a skin adnexal tumor, and after wide local excision and reconstruction, the diagnosis of trichilemmal carcinoma was confirmed. The patient recovered well, highlighting the importance of a thorough and structured diagnostic approach to prevent adverse outcomes. Regular follow-up is crucial for monitoring recurrence or metastasis, despite the lack of formal guidelines.
