TLDR Hormonal imbalances can cause heart rhythm issues, so checking hormone levels is crucial.
A 36-year-old female with panhypopituitarism, a condition involving the deficiency of all anterior pituitary hormones, experienced ventricular tachycardia (VT) due to severe adrenal insufficiency and thyroid hormone deficiency. These hormonal deficiencies led to electrolyte imbalances, causing QT interval prolongation and unstable cardiac electrical activity. The patient was treated with intravenous levothyroxine, hydrocortisone, electrolyte replacement, and fluid resuscitation, resulting in stabilization and discharge with improved QT interval. This case highlights the importance of assessing hormone levels, such as TSH and ACTH, alongside electrolytes when evaluating patients with ventricular arrhythmias, emphasizing the critical role of underlying endocrine pathology in cardiac dysfunction and arrhythmias.
