Long-Term Evaluation of Efficacy and Safety of Finasteride in Japanese Men with Androgenetic Alopecia: Summary of Three Investigations

    Masayuki Yanagisawa, Akio Sato
TLDR Finasteride is effective and safe for long-term use in treating hair loss in Japanese men.
Long-term treatment (over 10 years) with finasteride 1 mg/day in Japanese men with androgenetic alopecia (AGA) has shown high efficacy and safety. Three large-scale investigations (3,177 cases over 2.5 years, 801 cases over 5 years, and 532 cases over 10 years) demonstrated significant improvement in hair growth and prevention of disease progression, particularly in early-stage AGA patients and those younger than 40 years. Adverse reactions were minimal, with rates of 0.7% over 2.5 years and 6.8% over 10 years, and no cases of Post Finasteride Syndrome were reported.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
