This case report discusses a rare subtype of granular parakeratosis (GP) affecting the eccrine ostium, presented in a 64-year-old Thai female with a 40-year history of recurrent pruritic erythematous rashes on her neck. Initially misdiagnosed as eczema or folliculitis, the correct diagnosis of GP was confirmed through histopathological examination. The patient was advised to avoid excessive heat and keep intertriginous areas dry, leading to significant improvement in her condition. This highlights the challenges in diagnosing this rare condition and the importance of accurate identification for effective management.
