The Safety and Efficacy of Finasteride for Transgender Men with Androgenetic Alopecia: A Case Series

    TLDR Finasteride is safe and effective for hair loss in transgender men, but may affect testosterone therapy.
    This case series evaluated the safety and efficacy of finasteride in three transgender men with androgenetic alopecia undergoing testosterone replacement therapy in Japan. Over 6 months, participants aged 29, 43, and 44 were treated with finasteride, starting with 0.2 mg daily for 3 months, then 1.0 mg if no significant adverse events occurred. The study found finasteride to be safe, with no serious adverse events, and effective, as all participants showed improvement on the Norwood–Hamilton scale. However, two participants experienced resumption of menstruation, suggesting potential interactions with testosterone therapy that require further research. The study indicates finasteride may be a viable treatment for androgenetic alopecia in transgender men, but further investigation is needed to understand its impact on testosterone therapy.
