Finasteride: A New Drug for the Treatment of Male Hirsutism and Androgenetic Alopecia?

TLDR Finasteride may help reduce male hair loss and excessive hair growth by lowering DHT levels.
Finasteride was studied for its potential in treating male hirsutism and androgenetic alopecia by inhibiting the conversion of testosterone to dihydrotestosterone (DHT), particularly affecting type II 5 alpha-reductase. The drug showed promise in reducing hirsutism in women with hyperandrogenism and improving hair follicle characteristics in men with alopecia by increasing the number of follicles in the anagen phase and decreasing DHT levels in the scalp. This review summarized findings from recent studies and discussed the future potential of finasteride in these contexts.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

Finasteride and dutasteride have potential risks, including post-finasteride syndrome, but are commonly used for hair loss. RU58841, minoxidil, and needling are suggested as alternatives, though all treatments carry risks.

The conversation discusses the tension theory as a cause of male pattern baldness (MPB), suggesting that scalp tension and reduced subcutaneous fat layer contribute to hair loss. Treatments mentioned include Minoxidil, finasteride, microneedling, scalp massages, and Botox.
