Exosome-based regenerative therapies are advancing aesthetic medicine by improving skin aging, melasma, hair loss, and wound healing. These therapies utilize nanoscale extracellular vesicles to enhance intercellular communication, promoting fibroblast proliferation, collagen synthesis, and anti-inflammatory signaling. In hair restoration, exosomes increase hair density by 35–50 hairs/cm². The integration of nanotechnology and AI allows for enhanced delivery and personalized care, respectively. Despite promising results and minimal side effects, challenges such as exosome heterogeneity, high isolation costs, and lack of regulatory approval limit widespread adoption. Future directions include developing standardized protocols and conducting large-scale trials to ensure safe and effective clinical use.
