TLDR Test thyroid function in all obese patients and prioritize weight loss for hormonal balance.
The European Society of Endocrinology Clinical Practice Guideline on the Endocrine Work-up in Obesity, published in 2020, addressed the hormonal changes associated with obesity, which had a prevalence of approximately 20%. It recommended testing all obese patients for thyroid function due to the high prevalence of hypothyroidism. Hormonal testing for hypercortisolism, male hypogonadism, and female gonadal dysfunction was advised only when there was clinical suspicion of an underlying endocrine disorder. The guideline emphasized that weight loss should be prioritized to restore hormonal balance, noting that treating endocrine disorders had only a modest effect on weight loss.
