TLDR Diagnose PCOS in teens using irregular periods and high androgen levels, not ultrasound.
This literature review discussed the challenges of diagnosing polycystic ovary syndrome (PCOS) in adolescents due to the overlap with normal puberty events. It emphasized the need for specific diagnostic criteria, recommending the use of irregular menstrual cycles and hyperandrogenism as the main criteria, while excluding other conditions. The review highlighted the limited research on adolescents, particularly those "at risk of PCOS," and criticized the use of the Rotterdam criteria, which includes pelvic ultrasound, as inappropriate for adolescents. It suggested that anti-Müllerian hormone levels could support diagnosis if the main criteria are met.
