Recommendations on the Clinical Management of Androgenetic Alopecia: A Consensus Document from the Spanish Group of Trichology of the Spanish Academy of Dermatology and Venereology

    TLDR Dermatologists recommend oral dutasteride for male hair loss, low-dose oral minoxidil for female hair loss, and a multidisciplinary approach for young patients, with caution during pregnancy.
    A consensus statement developed by 34 dermatologists from the Spanish Hair Disorders Society recommends oral dutasteride 0.5 mg as the first-line therapy for male androgenetic alopecia (MAGA), due to its superior efficacy compared to finasteride 1 mg. For female androgenetic alopecia (FAGA), low-dose oral minoxidil is recommended over the topical formulation. The most effective treatment for FAGA in premenopausal women is a combination of minoxidil and a 5-alpha-reductase inhibitor or spironolactone, while in postmenopausal women, the combination of minoxidil and 5-alpha-reductase inhibitors is most effective. The consensus also suggests the use of procedures such as mesotherapy with platelet-rich plasma and low-level laser therapy, and advises a multidisciplinary approach for pediatric/adolescent patients. During pregnancy, most pharmacological treatments should be avoided, but specific nutricosmetics, low-power laser therapy, and PRP treatment can be considered if clinically necessary.
