TLDR Trichoscopy is crucial for diagnosing and managing androgenetic alopecia, showing increased vellus hairs, empty follicles, and fibrosis with severity.
The study developed and evaluated macroscopic and trichoscopic graded live visual scales for androgenetic alopecia (AGA) in men and women, correlating trichoscopic parameters with clinical severity. The study found that the rate of vellus hairs and empty follicles increased with AGA severity in both sexes, while single-hair follicular units (SHFUs) correlated with severity in women and the male frontal region but not the vertex. The peripilar sign did not show significant variation with severity. Anisotrichosis higher than 20% was essential for AGA diagnosis. The study highlighted the importance of trichoscopy in AGA classification and management, noting that fibrosis and hair coverage photodamage (HCPP) increased with disease severity.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
