A 30-year-old Korean woman presented with central trichoptilosis, characterized by longitudinal splitting of the hair shaft without tip involvement, and concomitant onycholysis. Her hair was dry and brittle due to extensive use of cosmetic agents, including permanent waves, hot-comb straightening, and hair dyes. Microscopic examination confirmed the hair shaft abnormalities. Despite previous unsuccessful treatments for presumed onychomycosis, the nail condition slightly improved after avoiding further cosmetic trauma. This case highlighted the rare occurrence of central trichoptilosis and associated nail changes due to external chemical exposure.
