Efficacy of Baricitinib in Treatment of Coexisting Alopecia Areata and Lichen Planopilaris

    TLDR Baricitinib effectively treated hair loss and inflammation in a patient with alopecia areata and lichen planopilaris.
    This case report discusses the successful use of baricitinib in treating a 41-year-old Black or African American female with coexisting alopecia areata (AA) and lichen planopilaris (LPP), two distinct types of alopecia. Despite previous treatments with topical and intralesional corticosteroids, the patient experienced progressive hair loss. After starting baricitinib, she showed noticeable hair regrowth and a reduction in inflammation without adverse effects. The case highlights the potential of baricitinib as an effective treatment for concurrent AA and LPP, particularly in patients with refractory conditions. It also emphasizes the importance of careful diagnosis and management of alopecia in Black or African American patients, who may present with multiple types of alopecia simultaneously. Further studies are needed to understand the pathogenesis and relationship between these conditions.
