Uncombable hair syndrome, also known as spun glass hair or pili trianguli et canaliculi, was characterized by dry, coarse blond to light-brown hair with a spangled appearance and unique hair shaft morphology. Initially described in 1973, it was typically congenital, with over 50 cases reported and some evidence suggesting an autosomal dominant inheritance. This document described the first case of acquired uncombable hair syndrome in a 39-year-old woman experiencing diffuse alopecia over three months, who was taking a combination oral contraceptive. Physical examination showed alopecia on the vertex and crown, along with minimal upper lip hirsutism.
