Vogt-Koyanagi-Harada Disease with Late-Onset Poliosis and Alopecia

This case report discusses a 55-year-old Japanese male with Vogt-Koyanagi-Harada (VKH) disease, a multisystem autoimmune disorder, who experienced late-onset poliosis and alopecia. Initially presenting with diffuse hair loss and white scalp hair, the patient had a history of bilateral uveitis at age 20. Despite treatment with triamcinolone acetonide injections and oral prednisolone, no hair regrowth was observed, but hair repigmentation occurred. The case is notable for the late onset of poliosis and alopecia, occurring 20 and 35 years after uveitis onset, respectively, which is unprecedented. The report highlights that reversal of poliosis and vitiligo in VKH can occur and may indicate a good prognosis, as seen in 27% of similar cases. The patient remains under follow-up for potential vitiligo development.
