This case study describes a 71-year-old postmenopausal woman with concurrent hypercalcemia and androgen excess, a rare combination that necessitates thorough diagnostic evaluation. The patient exhibited symptoms such as hirsutism, androgenic alopecia, and elevated androgen levels, alongside hypercalcemia and elevated parathyroid hormone levels, suggesting primary hyperparathyroidism. Imaging revealed a pelvic mass, raising suspicion of an androgen-secreting tumor, potentially of ovarian origin, like Sertoli-Leydig cell tumors. The case highlights the need for a comprehensive assessment to determine the underlying cause, considering possibilities like neoplastic, paraneoplastic, and genetic factors, including MEN1 syndrome. Pathologic evaluation of the mass is essential for diagnosis and guiding treatment.
