This case study discusses a 71-year-old postmenopausal woman with concurrent hypercalcemia and androgen excess, a rare combination that necessitates thorough diagnostic evaluation. The patient exhibited symptoms such as hirsutism, androgenic alopecia, and elevated androgen levels, alongside hypercalcemia and elevated parathyroid hormone levels, suggesting primary hyperparathyroidism. Imaging revealed a pelvic mass and ascites, indicating a potential ovarian tumor, possibly a Sertoli-Leydig cell tumor, while adrenal tumors were deemed unlikely. The case highlights the need for a systematic approach to diagnosing overlapping endocrine disorders, considering possibilities like paraneoplastic syndrome or MEN1. Pathological evaluation post-surgery is crucial for accurate diagnosis and management.
