TLDR Anastrozole can cause increased testosterone, leading to symptoms like hair growth and thinning.
This case study reports a rare instance of hyperandrogenism in a 74-year-old post-menopausal woman undergoing treatment with Anastrozole for ER-positive breast cancer. The patient developed symptoms such as hirsutism and scalp hair thinning, alongside elevated testosterone levels, which decreased after discontinuing Anastrozole. After ruling out other causes, it was concluded that Anastrozole likely caused the hyperandrogenism. The patient was switched to exemestane and treated with spironolactone for her symptoms. This case highlights the potential of Anastrozole, a selective aromatase inhibitor, to cause testosterone excess, a side effect not commonly described in post-menopausal women, and underscores the importance of excluding serious causes of hyperandrogenism before attributing it to aromatase inhibitors.
