A 74-year-old post-menopausal woman with a history of breast cancer treated with anastrozole developed hyperandrogenism, presenting as hirsutism and scalp hair thinning. Her elevated testosterone levels decreased after discontinuing anastrozole, suggesting the drug as the cause. Anastrozole, a non-steroidal aromatase inhibitor, can increase testosterone by inhibiting its conversion to estradiol. This case highlights the importance of ruling out serious causes of hyperandrogenism, such as ovarian hyperthecosis or tumors, before attributing symptoms to aromatase inhibitors. The patient was switched to exemestane and treated with spironolactone for her symptoms.
