A 60-year-old woman experienced virilization symptoms, including hirsutism, male pattern baldness, and acne, due to ovarian hyperthecosis, a rare condition in postmenopausal women. Despite normal levels of several hormones, her testosterone levels were elevated. A laparoscopic bilateral salpingo-oophorectomy was performed, revealing ovarian stromal hyperplasia with hyperthecosis and hilar and Leydig cell hyperplasia. Post-surgery, her testosterone levels decreased, and her symptoms improved significantly. This case underscores the importance of thorough evaluation in postmenopausal virilization, as ovarian hyperthecosis, though rare, can lead to significant androgen overproduction and associated complications.
