TLDR Tirzepatide improved symptoms and hair regrowth in a man with folliculitis decalvans.
This case report details a man in his 40s with a long-standing history of recalcitrant folliculitis decalvans (FD), who showed significant improvement in symptoms and hair regrowth after using tirzepatide for weight management. Previously ineffective treatments had failed to alleviate his condition, but tirzepatide, a GLP-1 receptor agonist, led to reduced pain, drainage, and flares, along with notable hair regrowth. These results suggest that tirzepatide may have beneficial anti-inflammatory and immunomodulatory effects for managing FD, indicating the need for further research into the therapeutic potential of GLP-1 agonists for FD and similar inflammatory skin conditions.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
