A woman in her 30s presented with androgenic alopecia, virilisation, and secondary amenorrhoea, and was found to have extremely high testosterone levels. Imaging revealed a right ovarian mass, leading to a diagnosis of a testosterone-secreting Sertoli-Leydig cell tumour, a rare type of ovarian neoplasm. The patient underwent right salphingo-oophorectomy, which resulted in normalisation of serum testosterone levels, reduction in virilisation, and resumption of menstrual cycles. She conceived spontaneously 2 months post-surgery.
