TLDR Early diagnosis and treatment stopped hair loss and improved the condition.
A clinical case of a 46-year-old patient with pseudopelades of Brocq, a form of scarring alopecia, was analyzed to determine optimal treatment strategies. The patient experienced hair loss over 12 months, with no clear cause, and presented with pale pink, smooth, atrophic skin in the balding areas. Trichoscopy revealed the absence of follicular openings and smooth white zones. Treatment included a peripheral vasodilator, B vitamins, corticosteroid lotion, and sensitive scalp shampoo. After 3 weeks, hair loss ceased, and no new areas appeared, with positive clinical dynamics observed over the next 4 weeks. Early diagnosis and treatment are crucial to prevent atrophy and effectively manage the condition.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.
