Microcystic adnexal carcinoma (MAC) is a rare, aggressive skin tumor that often recurs and can be easily misdiagnosed due to its clinical and histological similarities with other skin neoplasms. The study presents a case of a 61-year-old woman with recurrent MAC on her right eyebrow, which was successfully treated with total excisional surgery and A-T Flap surgery. After two years with no recurrence, hair transplantation using the follicular unit transplantation method was performed on the scarred area. The study concludes that complete surgical excision and long-term follow-up are necessary for managing MAC. Additionally, hair transplantation with the follicular unit transplantation technique can be a beneficial method for treating scars resulting from MAC excisional surgery.
