TLDR PRP and ADSC therapies show promise in improving symptoms of genital lichen sclerosus with minimal side effects.
This comprehensive review examines the use of platelet-rich plasma (PRP) and adipose-derived stem cells (ADSCs) as regenerative therapies for genital lichen sclerosus (LS), a chronic inflammatory condition affecting the genital area. Both PRP and ADSCs have shown promising results in improving LS-related symptoms such as itching, burning, and dyspareunia, while also allowing many patients to reduce or stop using topical corticosteroids. PRP promotes tissue regeneration and immune modulation, while ADSCs contribute through their plasticity and secretion of trophic factors. The therapies are generally safe, with minimal adverse effects. Combining PRP and ADSCs may offer a synergistic benefit, particularly in early-stage LS. Although current findings are encouraging, further research, including randomized controlled trials, is needed to fully understand their potential and mechanisms.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

Platelet-rich plasma (PRP) therapy for hair loss is expensive, painful, and often ineffective, with mixed reviews on its benefits. Alternatives like minoxidil, finasteride, and hair transplants are suggested as more reliable options.
