TLDR Larger prostate size in older men may be linked to poorer semen quality and fertility issues.
The study investigated the relationship between semen quality, lower urinary tract symptoms (LUTS), and prostate volume in aging men. A total of 316 men aged 45-67 were examined, with semen samples assessed according to WHO guidelines. The study found that sperm concentration and motility negatively correlated with total prostate volume and LUTS, while testicular size positively correlated with sperm concentration and motility. Additionally, seminal plasma interleukin-6 levels positively correlated with total prostate volume and PSA levels. The results suggest that semen pathologies and reduced fertility rates may be associated with LUTS and prostate diseases in aging men. However, future research is needed to directly define these relationships and examine the treatment effects of LUTS on fertility rates. The document also discusses the potential persistent adverse effects of 5α-reductase inhibitors, used to treat androgenetic alopecia and benign prostatic hypertrophy, on sexual function, depression, and quality of life, suggesting that physicians should exercise caution and thoroughly discuss these issues with patients before prescribing this therapy.
