TLDR Rare ovarian tumors can cause hyperandrogenism, even if imaging appears normal.
This case study describes a 66-year-old postmenopausal woman with hyperandrogenism and alopecia, ultimately diagnosed with rare ovarian tumors. Despite normal imaging, bilateral salpingo-oophorectomy (BSO) revealed bilateral Leydig cell tumors and a Brenner tumor. Post-surgery, her testosterone levels normalized, and alopecia resolved. This case highlights the diagnostic challenge of postmenopausal virilization and the necessity of surgical intervention for accurate diagnosis and treatment, especially when imaging does not reveal abnormalities.
