    TLDR A woman's increased facial hair and hair loss were caused by a rare ovarian tumor, cured by surgery.
    A 73-year-old postmenopausal woman presented with significant scalp hair loss and hirsutism, leading to social withdrawal. She had a high total testosterone level of 511 ng/dL, while other hormone levels were normal. An MRI revealed a 1.8 cm left ovarian nodule, diagnosed as a Leydig cell tumor. After bilateral salpingo-oophorectomy, her testosterone levels normalized, and her symptoms improved. Leydig cell tumors are rare, accounting for less than 0.1% of ovarian tumors, and are typically benign. Bilateral oophorectomy is recommended for postmenopausal women with suspected ovarian sources of hyperandrogenism.
