TLDR A 6-year-old girl with head lice developed scarring hair loss but improved with treatment.
A 6-year-old girl with pediculosis capitis presented with intense scalp pruritus, pustules, nodules, and alopecia in the occipital and temporal regions. Diagnosis was confirmed by identifying nits and adult lice on the hair shaft. Dermoscopic examination indicated cicatricial alopecia. Treatment with 1% permethrin lotion and antibiotics resulted in a good clinical response, with no lice or nits found at follow-up.
