Off-Label Use of Botulinum Toxin in Dermatology: Current State of the Art

    The document reviews the off-label use of Botulinum toxin (BoNT) in dermatology, discussing its effectiveness in treating various skin conditions. A study involving 20 patients with hidradenitis suppurativa showed significant improvement after BoNT-B injections. BoNT-A injections significantly reduced the severity of erythema in patients with rosacea. BoNT is also used in treating anal fissure, Raynaud Phenomenon, androgenetic alopecia (AGA), plaque psoriasis, inverse psoriasis, and Hailey-Hailey disease. In the case of AGA, two studies showed promising results with BoNT treatment, with one showing a significant increase in hair count after 24 weeks. BoNT-A has also been effective in treating conditions like Hailey-Hailey disease, eccrine nevus, oily skin, pompholyx, Darier’s disease, hypertrophic scars, and primary hyperhidrosis. However, more research is needed in many cases, and potential side effects of BoNT-A, such as pharyngitis and flu-like syndrome, are mentioned.
