This document describes a rare case of nevus comedonicus (NC) with terminal hair growth in a 26-year-old Caucasian male, which is unusual as NC typically presents as hairless lesions. The patient had comedones on his right arm, some containing pigmented hair stumps. Histopathological examination revealed a NEK9 mutation, which is associated with NC and affects follicular differentiation. This case challenges the prevailing understanding that NC lesions cannot develop terminal hair, suggesting that further research is needed to explore this phenomenon.
