Bilateral Nevus Comedonicus of the Eyelids

Pia R. Mendoza, Frederick A. Jakobiec, Daniel J. Townsend
TLDR A 79-year-old man was diagnosed with a rare skin condition called nevus comedonicus on his eyelids.
A rare case of late-onset, bilateral nevus comedonicus of the eyelids was reported in a 79-year-old man who presented with asymptomatic but disfiguring eyelid lesions. Physical examination revealed multiple papules resembling comedones on the eyelids, canthi, temple regions, and bridge of the nose. Microscopically, the lesions showed deep invaginations of the follicular canals forming tunnels or pseudosinus tracts with poral openings, lined by keratinizing and nonkeratinizing squamous epithelium, containing keratin lamellae, and some were inflamed. The diagnosis was confirmed as nevus comedonicus, and the report briefly discussed its clinical and histopathologic characteristics, pathogenesis, differential diagnosis, and management.
