TLDR Stopping minoxidil and using prostaglandin analogues improved the man's eye condition.
A 31-year-old male using topical minoxidil for androgenic alopecia for 8 years developed ophthalmic hypertension, presenting with diplopia and increased intraocular pressure (IOP) of up to 30 mm Hg. Examination revealed mild swelling of the bulbar conjunctiva and thinning of the ganglion cell layer. Discontinuation of minoxidil and treatment with prostaglandin analogues improved his condition. However, attempts to switch to carbonic anhydrase inhibitors failed, necessitating a return to the original hypotensive regimen. Over 10 months of observation, no progression was noted.
