TLDR Majocchi’s granuloma should be considered when rashes don't improve with typical treatments.
This case report describes a 54-year-old man with Majocchi’s granuloma, a rare fungal infection that can be mistaken for other skin conditions. The patient had a painful rash in the genitoinguinal area, which did not respond to various treatments. Although fungal cultures were negative, the clinical signs and history pointed to a deep fungal infection. A histopathological examination confirmed the diagnosis of Majocchi’s granuloma, which involves deep suppurative and granulomatous folliculitis. The patient was successfully treated with a 16-week course of itraconazole. The report highlights the need to consider Majocchi’s granuloma in differential diagnoses, even when topical antifungals have been ineffective.
