TLDR A 4-year-old girl had a rare hair disorder affecting only part of her scalp.
A 4-year-old girl exhibited uncombable hair syndrome (UHS) localized to a portion of her left scalp, while the rest of her hair remained straight and fine. Scanning electron microscopy revealed characteristics of pili trianguli et canaliculi, confirming the diagnosis. UHS is a rare hair shaft disorder typically affecting the entire scalp, but this case highlighted a localized variant. Diagnosis can be confirmed through microscopic examination showing longitudinal grooves and a triangulated cross-section of the hair shaft. Recognizing this localized variant is important for differential diagnosis in pediatric hair disorders.
