A Localized Rash in a 3-Year-Old Child

    A 3-and-a-half-year-old Japanese girl presented with itchy skin lesions on her left hip, initially treated with topical methylprednisolone with some improvement, but the lesions reappeared. Physical examination showed erythematous nodules and papules forming an indurated plaque, with a biopsy revealing a dense infiltrate of neutrophils, lymphocytes, and eosinophils. Initial tests for dermatophytes were negative, and treatment with hydrocortisone cream showed minimal improvement. Further examination revealed extensive suppurative folliculitis and a mixed granulomatous reaction in a hair follicle, leading to the diagnosis.
