Isotretinoin Treatment for Folliculitis Decalvans: A Retrospective Case-Series Study

Berna Aksoy, Aslı Hapa, Emine Mutlu
TLDR Oral isotretinoin, especially at higher doses for at least 3 months, is effective for treating folliculitis decalvans.
This retrospective case series study investigated the effectiveness of oral isotretinoin monotherapy in treating folliculitis decalvans (FD) in 39 male patients with a mean age of 37.9 years. Patients were treated with isotretinoin doses ranging from 0.1 to 1.02 mg/kg/day for a median duration of 2.5 months. The study found that 82% of patients achieved remission, with those receiving ≥0.4 mg/kg/day for ≥3 months showing better outcomes and a 66% non-relapse rate. The study concluded that oral isotretinoin monotherapy, particularly at doses ≥0.4 mg/kg/day for at least 3 months, could be a promising treatment for FD and suggested further research on this treatment approach.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

The post discusses the theory that persistent dandruff in areas prone to hair loss could be a sign of hair follicles dying due to DHT, indicating balding. The responses vary, with some users noting improvements in dandruff and hair loss with treatments like Dutasteride, while others experienced hair loss without dandruff.

Isotretinoin and tretinoin are different; isotretinoin can cause hair loss and has more side effects. Tretinoin may affect minoxidil absorption, and users suggest waiting before applying minoxidil.
