Infliximab Induced Lupus in Crohn's Disease

    A 59-year-old woman with Crohn's disease developed a lupus-like syndrome after receiving infliximab, a TNF-alpha inhibitor, due to intolerance to other treatments. She experienced multiple systemic symptoms, including hair loss, joint pain, and muscle weakness, and had positive ANA and dsDNA antibodies, indicative of infliximab-induced lupus. Despite discontinuing infliximab, her symptoms persisted, which is unusual for drug-induced lupus and suggests a possible underlying systemic lupus erythematosus (SLE) that was triggered by the medication. The case highlights the need for screening for autoimmune antibodies before starting TNF-alpha inhibitors to tailor treatment plans accordingly.
