TLDR Gene differences may affect baldness treatment response in Korean men.
This study analyzed the genetic polymorphisms of steroid 5α-reductase type 1 and 2 genes in Korean men with androgenetic alopecia. The results suggest that these polymorphisms may not be directly associated with the development of baldness or generation of different clinical phenotypes. However, there were racial differences in the distribution of polymorphisms for these two enzyme genes. The study also suggested that SRD5A1 and 2 gene polymorphisms might be associated with the therapeutic response to enzyme inhibitors such as finasteride. The SRD5A2 gene was found to be significantly associated with androgenetic alopecia in Korean men.

FCE 28260 (PNU 156765), an under-explored 5α-reductase inhibitor, showcases promising results in research by Giudici et al., outperforming well-known treatments like Finasteride in reducing the conversion of testosterone to DHT. Its superior efficacy, demonstrated through lower IC50 values in both natural and human recombinant enzyme studies, suggests it could offer more effective management of DHT-related conditions. Additionally, its lower molecular weight hints at better potential for topical application, potentially offering advantages in treating conditions such as androgenic alopecia. Despite its potential, it has not advanced in development, possibly due to financial limitations, leaving its therapeutic prospects and side effect profile largely unexplored.
