TLDR A 55-year-old woman with several health conditions did not see hair regrowth after a transplant.
In 2015, a 55-year-old African woman with a medical history of type 2 diabetes, hypothyroidism, hypertension, and depression sought medical attention for a 4-year history of frontotemporal alopecia and complete loss of eyebrows. She experienced scalp itching and occasionally developed red bumps on her limbs. Despite undergoing hair transplantation a year prior, she saw no improvement in her hair loss. Her medications included metformin for diabetes, levothyroxine for hypothyroidism, amlodipine for hypertension, and sertraline for depression. No other treatments for her hair loss had been attempted at the time of the report.

Scalp biopsies are crucial for diagnosing hair loss conditions like Diffuse Unpatterned Alopecia (DUPA) and retrograde hair loss, as treatments like finasteride and dutasteride may not be effective if other conditions are present. Combining PPAR-GAMMA agonists with retinoids could improve treatments for conditions like Lichen Planopilaris.

A 25-year-old experiencing hair thinning all over the body and head, despite using minoxidil and having low vitamin D levels, is advised to seek a second opinion from a dermatologist specializing in hair loss. The likely diagnosis is diffuse alopecia areata, which may require treatment with a JAK inhibitor.
