TLDR Finasteride use linked to increased suicidality in patients under 45.
A study published in JAMA Dermatology found a significant association between finasteride use for androgenetic alopecia and suicidality in patients under 45 years old. The study evaluated adverse events reported to WHO's Vigibase and found 356 reports of suicidality and 2926 reports of depression or anxiety in primarily male patients. The authors note that the disproportional signals "could result from adverse effects, such as persistent sexual dysfunction, having a greater toll on younger patients using finasteride to treat a nonurologic condition". Health care professionals are advised to conduct a full evaluation and a detailed, personalized risk-benefit assessment for patients before prescribing finasteride.

A user started taking finasteride for hair loss and asked about its effects, the use of Tretinoin for hair growth, the right time to start minoxidil, and the effectiveness of microneedling. The responses advised patience with finasteride, suggested minoxidil as a last resort, doubted microneedling's help with finasteride, and stated no need for microneedling with Tretinoin.

The individual is using oral minoxidil and finasteride to treat androgenetic alopecia with good results but is considering stopping due to concerns about long-term effects on the heart and potential hair dependency on the medication. They are also contemplating the impact of hair loss on gender dysphoria and considering the use of wigs as an alternative.
