This paper reported the first documented case of erythromelanosis follicularis faciei in a 17-year-old Saudi male, characterized by asymptomatic red-brown lesions with follicular papules on the face, decreased beard hair, and keratosis pilaris on the shoulders. The condition, which had been present for six years and aggravated by sun exposure, showed histopathological features such as mild hyperkeratosis, follicular plugging, and increased basal layer pigmentation. The etiology remained unknown, with possible genetic and environmental factors suggested. The case's clinical and histopathological features aligned with existing literature, except for the involvement of the auricles and lip, which might not be atypical due to the limited number of reported cases. Documenting this case was deemed important for local medical literature and differential diagnosis clarity.
